When it comes to CPAP mask fit, teeth matter more than you think.
If you’re missing teeth - especially if you have no natural lower teeth up front - getting a full-face mask to seal properly can be a challenge.
You may need to keep your mask really tight to get it to seal...but you may also find that it's going to leak and end up in your mouth. (That's not a fun way to wake up!)
You need a solid lower dental ridge to support mask fit with a full face mask.
One trick? Try wearing bottom dentures while using CPAP.
This can create a better seal, but don’t over-tighten the headgear—or you’ll pinch your lip against the denture, which can be painful.

Most people remove their dentures overnight to allow gum rest for at least four hours - at least that what the denture people recommend. If you decide to wear your teeth during sleep, make sure to give your gums a break at another time during the day.
Another option is to see if you can get away with a nose-only mask option (nasal mask or nasal pillows). These types of mask don't care if you sleep with teeth in or out to get a good seal.
While there's no universal perfect mask, finding the right balance between mask fit and oral comfort is key to a successful CPAP experience.
Why this topic?
It bothers me when I hear stories of CPAP users struggling because nobody thought about their smile when discussing mask options with them.
And then, when their mask is leaking and blowing like a Category 5 Hurricane, the question of dentures during sleep is not top-of-mind.
Because of this, I've included it as part of my Free CPAP Mask Tip Sheets. These sheets are full of common sense wisdom you can use right away, rather than having to figure it out on your own.
